Skilled Services

What are Skilled Services?

Seasons of Care provides Skilled Services when a person comes home from the hospital or rehabilitation facility. There are also situations when the person is “homebound” and the physician decides that Skilled Home Care Services are necessary to help them recuperate from an injury or illness.

Seasons of Care provides compassionate and expert Skilled Nursing and Therapy Services. Perhaps a Nurse is needed to dress the wound of a patient who has just returned home from the hospital after surgery. Or a person needs Occupational Therapy in order to perform their activities of daily living after a stroke.

Our Physical Therapists, Occupational Therapists and Speech Language Pathologists work closely with patients recovering from orthopedic or cardiac surgery, injuries, neurological disorders, and many other types of health problems. They help people achieve their maximum level of functioning. Our goal is to help patients to return to as full and productive life as possible.

  • Physical Therapy: Through therapeutic exercise, balance activities and ambulation training, our physical therapists help patients regain functional mobility and gross motor skills.
  • Occupational Therapy: Using fine motor, cognitive, perceptual and sensory exercises and tasks, our occupational therapists help patients regain daily living skills.
  • Speech Language Pathology: Our speech language pathologists help patients regain their ability to produce and understand speech as well as facilitate communication skills. They also help patients manage swallowing disorders.

All of our rehabilitation therapists are state-licensed professionals. Many have also earned advanced degrees in their specific fields. Each therapist works with the patient's physician, home health care nurse, and other disciplines to ensure that comprehensive care is provided.

Benefits of In-Home Rehabilitation Therapy

Even though a patient may have had Therapy in the hospital or a  nursing home after surgery or an illness, they often need to continue Therapy at home for several reasons.  First, the goal of the In-Patient Therapy is to get the patient strong enough to return home. When they do go home, they often need continued Therapy Services to maximize their functional ability so they can be safe and independent at home.

There are many benefits to providing high-quality rehabilitation in the comfort of a patient's own home:

  • The therapist can assess the patient's needs within the context of context of the patient's home.
  • The therapist can identify safety concerns and environmental barriers, determine if there's a need for adaptive equipment and therapeutic intervention, plus evaluate the patient's ability to function within his or her environment.
  • The patient's family members and friends can play an active and vital role in the patient's therapy, assisting with exercises and providing ongoing encouragement.

When a person comes home from the hospital or rehabilitation facility, our therapists “pick up” where the inpatient facility therapists ended. This level of continuity significantly enhances and quickens the patient’s recovery.

Skilled Services are usually covered by Medicare.
